What You'll Do

As a Hospice RN Case Manager, you will serve as the primary nurse coordinating care for patients and supporting families through every stage of the hospice journey.

Clinical Responsibilities

• Manage a caseload of hospice patients in private homes, assisted living communities, and skilled nursing facilities
• Perform comprehensive nursing assessments and ongoing evaluations
• Develop, implement, and update individualized plans of care
• Manage pain and symptom control to maximize patient comfort and quality of life
• Provide education and emotional support to patients and their families regarding disease progression and end-of-life care
• Coordinate care with physicians, social workers, chaplains, CNAs, Home Health Aides, and the hospice interdisciplinary team
• Supervise Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs)
• Maintain accurate and timely documentation within the Electronic Medical Record (EMR)
• Ensure compliance with Medicare, state, and organizational hospice regulations

Qualifications

Required

• Active North Carolina Registered Nurse (RN) license
• Current CPR certification
• Valid driver's license
• Reliable transportation with current automobile insurance
• Strong assessment, communication, and time management skills

Preferred

• Minimum of one year of RN experience
• Hospice experience
• Home Health experience
• Palliative Care experience
• Case Management experience
• Experience using Electronic Medical Records (EMR)
