Responsibilities

As a Hospice RN Case Manager, you will:

• Manage a caseload of hospice patients in homes, assisted living communities, and skilled nursing facilities
• Complete comprehensive nursing assessments
• Develop, implement, and update individualized plans of care
• Manage pain and symptom control
• Coordinate care with physicians, social workers, chaplains, and other members of the hospice interdisciplinary team
• Educate patients and family members regarding disease progression and comfort care
• Supervise Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs)
• Maintain accurate electronic medical records (EMR)
• Ensure compliance with Medicare, state, and company standards

Qualifications

• Active North Carolina Registered Nurse (RN) License
• Current CPR Certification
• Minimum one year of RN experience preferred
• Hospice, Home Health, Palliative Care, or Case Management experience strongly preferred
• Valid driver's license
• Reliable transportation with current automobile insurance
• Strong assessment, communication, and time management skills
• Experience using Electronic Medical Records (EMR) preferred
