We are seeking an experienced and compassionate Hospice RN Case Manager to join our interdisciplinary team. This role is ideal for a nurse who values autonomy, relationship-based care, and making a profound impact on patients and families.


What You’ll Do


As an RN Case Manager, you will oversee and coordinate the total plan of care for a caseload of hospice patients in home and facility settings.


Clinical Leadership
  • Perform comprehensive patient assessments
  • Develop and update individualized Plans of Care
  • Manage symptoms and provide end-of-life education
  • Coordinate care with physicians and hospice Medical Director
  • Supervise Hospice Aides / Home Maker Aides
  • Ensure compliance with Medicare Conditions of Participation
  • Participate in IDG meetings
Care Coordination
  • Collaborate with social workers, chaplains, aides, and therapy services
  • Provide education and emotional support to patients and families
  • Facilitate transitions in care levels (Routine, GIP, Respite, Continuous)
  • Ensure timely documentation and regulatory compliance
Documentation & Compliance
  • Complete timely and accurate EMR documentation
  • Maintain audit-ready clinical records
  • Follow agency policies and state/federal hospice regulations
Qualifications
  • Active, unrestricted Virginia RN license
  • 1–2 years of hospice experience preferred
  • Strong assessment and symptom management skills
  • Knowledge of Medicare hospice regulations
  • Current CPR certification
  • Valid driver’s license and reliable transportation
  • Excellent communication and time management skills
