About the Role: Solace Hospice & Palliative Care Inc is looking for a compassionate and dedicated Hospice RN Case Manager to join our team in Saint Charles, IL. In this meaningful role, you'll be at the heart of delivering exceptional end-of-life care, guiding patients and their families through one of life's most important journeys. Responsibilities:
- Manage a caseload of hospice patients, conducting comprehensive assessments and developing individualized care plans
- Perform skilled nursing visits in patient homes, skilled nursing facilities, and other care settings throughout the Saint Charles area
- Coordinate interdisciplinary team (IDT) care, collaborating with physicians, social workers, chaplains, and aides
- Attend every-other-week Interdisciplinary Team (IDT) meetings (in person or via phone/video) to review patient plans of care, evaluate progress, and collaborate on care coordination
- Monitor and manage pain and symptom control, adjusting care plans as patient needs evolve
- Educate patients and families on disease progression, comfort care, and end-of-life planning
- Maintain accurate and timely clinical documentation in compliance with hospice regulations and Medicare guidelines
- Participate in the scheduled on-call rotation to provide coverage for urgent patient and family needs after hours and on weekends
Requirements:
- Active RN license in the state of Illinois, in good standing
- Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
- Strong knowledge of Medicare hospice regulations and care standards
- Excellent clinical assessment, pain management, and symptom control skills
- Exceptional communication and interpersonal skills with patients, families, and care teams
- Ability to work independently, manage time effectively, and prioritize a dynamic caseload
- Valid driver's license and reliable transportation for field-based visits
