Job Summary
The Primary Hospice RN provides compassionate, patient-centered care to individuals with serious illness in the place they call home, including private residences, nursing homes, assisted living, and group homes. The RN manages an assigned caseload, leads the interdisciplinary care team, provides expert symptom management, educates patients and caregivers, and helps patients achieve their personal goals of care.
Key Responsibilities
Manage an assigned hospice patient caseload.
Provide comprehensive nursing assessments and symptom management.
Educate and support patients and caregivers in delivering safe, confident care.
Collaborate with the interdisciplinary team to develop and coordinate individualized care plans.
Deliver hospice care in home and residential care settings.
Schedule
Monday–Friday, 8:00 AM–5:00 PM
Occasional weekend backup and on-call coverage
Flexible daily scheduling to support work-life balance
