Responsibilities

  • Provide high-quality Hospice nursing care to patients in Marysville, CA

  • Assess patient needs, develop and update individualized care plans

  • Administer medications and monitor for side effects and efficacy

  • Monitor patient symptoms and implement comfort measures to improve quality of life

  • Coordinate with interdisciplinary team to ensure holistic patient support

  • Document care activities accurately in electronic health records

  • Educate patients and families on end-of-life care, procedures, and expectations

  • Ensure compliance with healthcare regulations, safety, and ethical standards

  • Utilize performance metrics and pipelines to track patient outcomes and care delivery

  • Escalate high-risk issues and coordinate rapid response when needed

  • Participate in ongoing professional development and seek opportunities for advancement


Experience/Qualifications

  • Registered Nurse license in California with Hospice/Home Health experience

  • Proven ability to develop and update individualized care plans for end-of-life patients

  • Administer medications, monitor side effects, and adjust plans accordingly

  • Strong symptom assessment and comfort-measure expertise for palliative care

  • Knowledge of healthcare regulations, safety, and ethical standards in hospice

  • Experience coordinating with interdisciplinary teams and care coordination

  • Accurate documentation in electronic health records and data integrity

  • Patient and family education on end-of-life care and expectations

  • Respond quickly to high-risk issues with escalation and rapid response

  • Ability to use performance metrics and pipelines to track outcomes

  • Commitment to ongoing professional development and advancement
