Responsibilities
Provide high-quality Hospice nursing care to patients in Marysville, CA
Assess patient needs, develop and update individualized care plans
Administer medications and monitor for side effects and efficacy
Monitor patient symptoms and implement comfort measures to improve quality of life
Coordinate with interdisciplinary team to ensure holistic patient support
Document care activities accurately in electronic health records
Educate patients and families on end-of-life care, procedures, and expectations
Ensure compliance with healthcare regulations, safety, and ethical standards
Utilize performance metrics and pipelines to track patient outcomes and care delivery
Escalate high-risk issues and coordinate rapid response when needed
Participate in ongoing professional development and seek opportunities for advancement
Experience/Qualifications
Registered Nurse license in California with Hospice/Home Health experience
Proven ability to develop and update individualized care plans for end-of-life patients
Administer medications, monitor side effects, and adjust plans accordingly
Strong symptom assessment and comfort-measure expertise for palliative care
Knowledge of healthcare regulations, safety, and ethical standards in hospice
Experience coordinating with interdisciplinary teams and care coordination
Accurate documentation in electronic health records and data integrity
Patient and family education on end-of-life care and expectations
Respond quickly to high-risk issues with escalation and rapid response
Ability to use performance metrics and pipelines to track outcomes
Commitment to ongoing professional development and advancement
