Our client is seeking a Hospice RN to join a dedicated, supportive care team focused on delivering comfort, dignity, and quality of life to patients and their families.
What You’ll Do
Manage and deliver individualized hospice care plans
Provide compassionate, patient-centered care in home settings
Support patients and families through education and guidance
Complete timely, accurate documentation using EMR systems
Coordinate with an interdisciplinary team to ensure continuity of care
What We're Looking For
Active RN license in Pennsylvania
Associate’s degree or higher in Nursing
2+ years of RN experience (hospice experience strongly preferred)
Experience with EMR systems (TruChart or similar)
Strong time management and ability to work independently
Valid driver’s license, reliable transportation, and auto insurance
Schedule Monday–Friday, 8:00am–5:00pm On-call rotation: one weekend every 5–7 weeks (Fri–Sun, 8pm–8am) Two holidays per year
