Job Summary

Hospice RN

Full-time, Contract, 13 weeks, Day Shift, 36 hours a week.


Responsibilities

  • Provide comprehensive hospice nursing care to patients in their homes or facility.

  • Maintain compliance with regulatory standards and hospice program policies.

  • Assess patient needs, develop care plans, and adjust based on condition changes.

  • Administer medications and manage symptom relief in alignment with palliative guidelines.

  • Document patient status, care provided, and outcomes in electronic medical records.

  • Engage in ongoing professional development to advance clinical skills and leadership potential.

  • Educate patients and families on disease trajectory, care options, and comfort measures.

  • Collaborate with interdisciplinary team to coordinate care and smooth handoffs.

  • Identify and report changes in patient conditions to supervising clinicians promptly.

  • Meet performance targets using care metrics and maintain high-quality outcomes.

  • Provide bereavement support referrals and follow-up as per protocol.

  • Demonstrate fiscal responsibility by optimizing resource use and avoiding unnecessary tests.


Experience/Qualifications

  • Registered Nurse with current license and Hospice/Palliative care certification preferred.

  • Experience delivering home health or hospice nursing with symptom management.

  • Ability to develop and adjust patient-centered care plans and escalation protocols.

  • Strong knowledge of palliative guidelines, medications, and safety in symptom relief.

  • Proficiency in electronic medical records and documentation for care plans and outcomes.

  • Effective communication with patients, families, and interdisciplinary teams.

  • Ability to meet performance targets using care metrics and quality outcomes.

  • Commitment to continuous professional development and leadership growth.

  • Bereavement support knowledge and referral process for families.

  • Demonstrated fiscal responsibility and resource optimization in care delivery.
