What You'll Do
- Complete comprehensive psychosocial assessments for hospice patients and families.
- Provide emotional support, counseling, crisis intervention, and grief support throughout the hospice journey.
- Help families understand available resources, community services, financial assistance, and advance care planning.
- Advocate for patients while ensuring their emotional, cultural, and psychosocial needs are respected.
- Collaborate closely with nurses, physicians, chaplains, hospice aides, and the interdisciplinary team to provide holistic care.
- Support family meetings, care planning, discharge coordination when appropriate, and bereavement planning.
- Maintain timely, accurate documentation while ensuring compliance with hospice regulations and Medicare Conditions of Participation.
- Be a trusted source of guidance, reassurance, and compassion during life's most meaningful moments.
What We're Looking For
- Bachelor's or Master's Degree in Social Work (BSW or MSW).
- Hospice experience is required.
- Strong understanding of grief, loss, end-of-life care, and family dynamics.
- Exceptional communication, active listening, and relationship-building skills.
- Ability to work independently while thriving as part of an interdisciplinary team.
- Compassionate, dependable, organized, and emotionally resilient.
- Passion for serving patients and families with dignity, empathy, and respect.
