What You’ll Do<\/span>
<\/p>

As a Hospice Social Worker, you'll collaborate with physicians, nurses, therapists, chaplains, and volunteers to provide compassionate psychosocial support, advocate for patients and families, and help develop individualized plans of care throughout the hospice journey.<\/span>
<\/p>

Key Responsibilities<\/span>
<\/p>

  • Conduct comprehensive bio -psychosocial assessments and ongoing reassessments<\/span>
    <\/li>
  • Provide emotional support, education, and counseling patients and families<\/span>
    <\/li>
  • Advocate for patients while coordinating care with the interdisciplinary hospice team<\/span>
    <\/li>
  • Participate in daily team huddles and interdisciplinary group (IDG) meetings<\/span>
    <\/li>
  • Assist with advance directives, POLST forms, medical power of attorney, and guardianship<\/span>
    <\/li>
  • Connect patients and families with community resources, transportation, respite care, and long -term care services<\/span>
    <\/li>
  • Help patients and families access financial assistance programs, disability benefits and insurance resources including VA, SSI, Medicare, Medicaid and Private Insurance options<\/span>
    <\/li>
  • Provide bereavement support and referrals for grief counseling<\/span>
    <\/li>
  • Coordinate with chaplains, healthcare facilities, and community partners to meet patient needs<\/span>
    <\/li>
  • Educate and guide patients and families in understanding medical information, treatment plans, and available care options to support informed decision -making<\/span>
    <\/li>
  • Set meaningful goals, ensure adherence to care plans, and address barriers that may impact appropriate treatment<\/span>
    <\/li>
  • Promote hospice philosophy while maintaining professional growth and best practices<\/span>

    What We’re Looking For<\/span>
    <\/p>

    • Master’s degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE)<\/span>
      <\/li>
    • Current Montana social work license (LSW or LCSW) or actively working toward licensure<\/span>
      <\/li>
    • Minimum of one year of social work experience in a healthcare setting required<\/span>
      <\/li>
    • Hospice, home health, palliative care, or community -based care experience preferred<\/span>
      <\/li>
    • Knowledge of chronic disease management, end -of -life care, and psychosocial support services preferred<\/span>
      <\/li>
    • Strong communication, counseling, advocacy, and care coordination skills<\/span>
      <\/li>
    • Valid driver’s license and ability to travel within the service area<\/span>
      <\/li><\/ul>
