Hospice Social Worker

We are seeking a dedicated and compassionate Hospice Social Worker to join our interdisciplinary hospice team and provide emotional support, advocacy, and guidance to patients and families during one of life's most meaningful journeys.

What You'll Do
- Conduct comprehensive psychosocial assessments to identify the emotional, social, environmental, and financial needs of patients and families.
- Develop individualized plans of care in collaboration with patients, caregivers, and the interdisciplinary hospice team.
- Provide emotional support and counseling related to illness, grief, and end-of-life planning.
- Facilitate conversations regarding goals of care and advance care planning.
- Connect patients and families with community resources and services to support their needs.
- Advocate for patients and caregivers to ensure they receive compassionate, patient-centered care.
- Collaborate closely with nurses, physicians, chaplains, and other members of the hospice team to provide holistic care.
- Participate in quality improvement initiatives and maintain compliance with agency policies and professional standards.
- Support patients and families through transitions and changes in care needs.
- Maintain timely and accurate documentation.
What We're Looking For
- Bachelor's degree in Social Work required.
- Master's degree in Social Work (MSW) preferred.
- Knowledge of hospice, palliative care, or healthcare social work is preferred.
- Strong communication, counseling, and interpersonal skills.
- Ability to work independently while collaborating effectively within an interdisciplinary team.
- Compassionate, empathetic, and committed to providing exceptional patient and family care.
- Strong organizational and time-management skills.
- Commitment to the values and ethical standards of the social work profession.
