Part-Time Hospice Social Worker (MSW)Join our dedicated team!We are seeking a compassionate, skilled, and mission-driven Social Worker (MSW) to join our small and dedicated hospice team. This part-time position offers a unique opportunity to provide meaningful support to patients and families during one of life's most important journeys while working alongside a close-knit interdisciplinary team that values collaboration, respect, and individualized care.If you are passionate about helping others navigate complex emotional, psychosocial, and practical challenges with dignity and compassion, we would love to hear from you.About the RoleAs a Hospice Social Worker, you will play an essential role in supporting patients and their loved ones through the emotional, social, and practical aspects of end-of-life care. You will provide counseling, advocacy, resource coordination, and guidance while helping patients and families find comfort, meaning, and support during a challenging time.Key ResponsibilitiesConduct psychosocial assessments of patients and familiesProvide emotional support, counseling, bereavement and crisis intervention as neededAssist patients and caregivers with advance care planning and end-of-life decision-makingConnect families with community resources and supportive servicesFacilitate discussions regarding patient goals, family dynamics, coping, and bereavement needsCollaborate closely with nurses, physicians, chaplains, aides, and other members of the hospice interdisciplinary teamAdvocate for patient and family needs while promoting dignity, comfort, and quality of lifeComplete timely and accurate documentation in accordance with hospice standardsReporting Relationship: Executive DirectorQualifications: Masters Degree in Social Work from a CSWE accredited program. Licensed as a Social Worker through the Massachusetts Board of Social Worker. Three years recent (within the past five years) experience in acute, long-term care or home care/hospice, with at least one in hospice or homecare preferred. Knowledge of community resources including Medicare and Medicaid, financial aid, county, state, and private social services agencies.Strong communication, counseling, and interpersonal skillsAbility to work independently while remaining an engaged member of a multidisciplinary teamCompassionate, empathetic, and patient-centered approach to careWe are committed to finding someone who shares our values and passion for hospice care, and we are open to creating a role that supports both the needs of our patients and the strengths of the ideal candidate.
