Hospice Team Director - Clinical Manager

Introduction:

Our client, a leading organization in the healthcare industry, is recognized for its unwavering commitment to compassionate hospice care, clinical excellence, and meaningful patient experiences. We are seeking a dedicated Hospice Team Director—an influential leadership role ideal for a clinical professional motivated to shape the future of hospice care by fostering collaborative teams and promoting exceptional quality.

Key Responsibilities:

- Lead interdisciplinary care teams—including nurses, physicians, social workers, and spiritual care providers—to deliver comprehensive, patient-centered hospice care addressing physical, emotional, social, and spiritual needs.

- Ensure adherence to best practices in hospice delivery, maintaining compliance with regulatory standards and organizational policies.

- Champion quality improvement initiatives, introduce innovative care solutions, and drive ongoing excellence in service delivery.

- Oversee daily clinical operations, including caseload management, resource allocation, and productivity to ensure operational effectiveness.

- Provide mentorship, coaching, and professional development opportunities to clinical staff, cultivating a supportive and collaborative team culture.

- Collaborate on initiatives aimed at enhancing patient outcomes, improving patient and family experiences, and advancing hospice care.

Requirements:

- Bachelor’s degree in Nursing (BSN) or a related health/human services field from an accredited program; equivalent experience considered.

- A minimum of three years’ clinical experience in hospice care is required.

- Strong knowledge of hospice care delivery, quality improvement processes, and regulatory compliance standards.

- Demonstrated leadership experience managing interdisciplinary clinical teams with a collaborative and empathetic approach.

- Excellent communication, organizational, and leadership skills.

- Clinical management experience preferred.
