Job Description

RN Case Manager

13 weeks, 5x8 08:00-16:30 Monday-Friday

Requirements:
  • Must be a MD RN
  • Acute rehabilitation, discharge planning, interdisciplinary collaboration, and care coordination are required
  • Acute Hospital experience required
  • Must have completed at least 1 travel assignment
  • BLS required
(This facility requires that all employees have the Flu vaccine*)
