What Youll Do
- Serve as a vital liaison between hospital discharge teams and respiratory/DME services.
- Coordinate respiratory and durable medical equipment (DME) needs for patients preparing for discharge.
- Facilitate timely delivery and setup of necessary medical equipment at hospitals and patient homes.
- Educate patients and caregivers on proper equipment use, safety, and maintenance.
- Collaborate closely with case managers, social workers, nurses, and physicians to ensure smooth patient transitions.
- Maintain accurate referral documentation, insurance verification, and activity records.
- Troubleshoot equipment issues and assist hospital staff with service-related questions.
- Participate in quality improvement initiatives and healthcare compliance programs.
- Travel to patient homes for equipment setup and follow-up as needed.
What You Need
Required:- High school diploma or GED
- Valid drivers license with a clean driving record
- Strong communication and relationship-building skills
- Ability to work effectively inside a hospital environment
