What Youll Do
Collaborate with hospital staff (case managers, nurses, physicians, discharge planners) to coordinate patient care
Ensure timely delivery, setup, and education on respiratory and DME equipment
Verify insurance and complete referral documentation
Maintain accurate patient records and documentation systems
Support emergency equipment requests and troubleshoot issues promptly
Participate in quality improvement initiatives and ensure compliance with standards
Provide in-service education and CEU presentations
Travel to patient homes for equipment setup and follow-up using a company vehicle
What You Need
Required:
High school diploma or GED
Valid drivers license with a clean driving record
Strong communication and organizational skills
Ability to lift and transport equipment up to 65 pounds
Proficiency with Microsoft Outlook, Word, Excel, and electronic documentation systems
Preferred:
Healthcare or medical equipment experience
Knowledge of insurance verification and medical billing
3+ years of healthcare experience
Experience working with hospital teams and patient coordination
About the Assignment
This role acts as a key link between hospital discharge teams and home healthcare services. You'll work inside two local hospitals, coordinating respiratory and DME services to facilitate safe patient transitions from hospital to home. The position involves direct patient interaction, hospital collaboration, and minimal travel, with no on-call duties.
