Are you passionate about making a difference in the healthcare industry and eager to help those affected by substance use disorders, addiction, and behavioral health issues? We are seeking a motivated and experienced Consulting Hospital Patient Navigator/Administrative Assistant who is committed to providing exceptional patient care and service. This role is crucial in navigating patients through the healthcare system and ensuring they receive the proper care and support they need. The ideal candidate will have extensive experience with Epic, patient outreach, care coordination, and working with underserved populations.

Responsibilities:

• Serve as a primary point of contact for patients, providing them with guidance and support throughout their healthcare journey.
• Utilize Epic Hyperspace for scheduling and tracking patient appointments and progress.
• Engage in patient outreach activities, ensuring patients are informed of their appointments, procedures, and any other necessary information.
• Coordinate patient care, working closely with healthcare professionals to ensure patients receive the proper treatments and services.
• Provide administrative support to the healthcare team, including managing patient records, handling correspondence, and maintaining organized and efficient office operations.
• Build and maintain strong relationships with patients, particularly those from underserved populations, using strong communication skills and a compassionate approach.
• Work proactively to identify any barriers to care and work with patients and healthcare professionals to address these issues.

Qualifications:

• Minimum of 5 years of experience in a healthcare administrative role.
• Experience working with Epic and Epic Hyperspace scheduling.
• Experience in patient outreach, engagement, and care coordination.
• Prior experience working with underserved populations, ideally individuals affected by substance use disorders (SUD), addiction, recovery, harm reduction, homelessness, or behavioral health.
• Strong administrative skills, including experience in a front desk or patient access role.
• Exceptional communication skills, with the ability to build trust and rapport with vulnerable populations.
• A compassionate and empathetic approach to patient care.
• Ability to navigate complex healthcare systems and processes.
• Strong problem-solving skills and the ability to work proactively to address any issues or barriers to care.
• Commitment to providing exceptional patient care and service.
• Ability to maintain confidentiality and handle sensitive information with discretion.
