About the Role:AT HOME CARE PHYSICIANS PLLC is looking for a compassionate and driven House Call Nurse Practitioner to join their team in Warrenville, IL. Responsibilities:- Conduct comprehensive in-home patient assessments, evaluations, and follow-up visits
- Diagnose and manage acute and chronic conditions for homebound patients
- Develop and implement individualized care plans in collaboration with physicians and care teams
- Prescribe medications and order diagnostic tests as appropriate within scope of practice
- Coordinate with specialists, home health agencies, and other healthcare providers to ensure continuity of care
- Maintain accurate and timely clinical documentation in the EMR system
- Educate patients and families on disease management, medications, and preventive care
Requirements:- Active Nurse Practitioner license in the state of Illinois
- Master's degree or higher in Nursing (MSN, DNP) with NP certification (FNP, GNP, or AGNP preferred)
- Experience in primary care, geriatrics, or home-based care strongly preferred
- Valid DEA license and ability to prescribe medications
- Reliable transportation and valid driver's license for travel to patient homes
- Strong clinical assessment and critical thinking skills in independent settings
- Excellent communication, empathy, and patient-centered bedside manner
