Position Summary:Vita Nova is seeking a compassionate and resourceful Housing Case Manager to support implement and administer its Permanent Supportive Housing (PSH) program for young adults with documented disabilities who are experiencing homelessness. Using a trauma-informed approach, the Housing Case Manager partners with program participants to develop individualized case plans, provide intensive housing-focused case management and life skills training, coordinate community resources, and provide support for the achievement of housing stability, improved social and emotional well-being, and pathways toward self-sufficiency. This position works both in the office and throughout the community, and includes occasional evening, weekend, and rotational on-call responsibilities.
