They will employ an integrated housing and care management service delivery model to improve housing and health outcomes for people living with HIV, with a specialized focus on adults age 55+.The OAHCM provides intensive case management, including permanent housing placement (PHP) and short-term rental, mortgage, and utility assistance (STRMU), to adults ages 55+ and any other eligible participant in need of these services. Working with Chicago House staff, external medical providers, landlords, and community-based agencies, the OAHCM addresses barriers related to social determinants of health and supports participants in obtaining and maintaining stable housing.
