Schedule-Monday-Friday, 40 hours weekly.

The Housing Liaison helps individuals access and maintain stable housing by connecting them with housing programs, community resources, and supportive services. This role works collaboratively with consumers, clinical and case management teams, landlords, and community partners to create pathways toward housing stability and greater independence. Through a strengths-based, Housing First approach, the Housing Liaison helps individuals navigate resources, overcome practical barriers, and connect with services that support their personal goals and overall wellness.

You’ll serve as a trusted resource and community connector, collaborating with landlords and local organizations while helping individuals access the supports and opportunities that can strengthen their housing stability and quality of life.

Key Responsibilities

  • Connect individuals and care teams with housing programs, benefits, and community resources that support housing stability.
  •  Provide person-centered outreach, engagement, education, and resource navigation for individuals seeking housing support. 
  •  Build strong relationships with landlords and community partners to expand housing opportunities and support successful tenancy. 
  •  Coordinate referrals and connections to behavioral health, substance use, physical health, employment, benefits, and other supportive services. 
  •  Support individuals with practical needs related to housing, including identification documents, benefits applications, transitions to permanent housing, and reengagement with services. 
  •  Apply Housing First, harm reduction, and other low-barrier approaches that meet individuals where they are and support their goals. 
  •  Collaborate with local Continuum of Care partners and participate in coordinated entry, case conferencing, and housing-focused initiatives. 
  •  Serve as a knowledgeable resource to individuals, case managers, clinical teams, and agency leadership regarding housing and community resources. 
  •  Maintain timely documentation and data collection in accordance with agency and Missouri Department of Mental Health requirements. 
  •  Participate in required statewide housing training, meetings, and professional development.

Requirements, Skills, Knowledge and Expertise

  • High School/GED required
  •  Bachelor’s degree in social work, sociology, psychology or related field preferred 
  •  Three (3) years relevant case management experience in housing required if Bachelor’s degree is not obtained 
  •  A valid, unrestricted driver’s license and agency established minimum automobile coverage required
