Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Our Agency oversees 20 residential settings, two outpatient offices, three adult training facilities, a prevocational enterprise program, a consumer drop-in center, habilitation services, and two Health Care Quality Units in Butler and Warren, which train professionals in 18 counties. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.

 

 

Responsibilities:

The Housing Mentor, reports to the Director of Housing Support, and is responsible for showing apartments and bedrooms to prospective renters, leasing units to applicants or referral holders, renewing leases for current tenants, and performing necessary managerial duties for the property. Responsibilities also include collaboration with the client’s treatment team to address any concerns and document relevant information in applicable systems.  Additionally, the Housing Mentor ensures that clients adhere to their tenant agreements.

 

    • This position will support all tenants in the program across all rental units as the needs arise
    • Exemplifies an attitude and behavior that models the organization’s values to tenants, service providers, regulatory agents, and the community at large 
    • Maintains professional contact with treatment teams and the community as required
    • Screening of referrals of potential tenants 
    • Assist clients with orientation to the living environment
    • Assist clients with transitioning to another housing environment
    • Responsible for monitoring tenants’ living space for health, safety and welfare concerns
    • Maintains a cost-effective building with an established budget
    • Responsible for submission of the weekly reporting requirement on the Synergy platform located on the Allegheny County DHS website
    • Reporting significant events to the Director of Housing Support
    • Responsible for coordinating and implementation of monthly/quarterly safety and fire drills
    • Checks fire systems monthly
    • Responds to building emergencies promptly
    • Adhere to the Fair Housing Act and local renting laws
    • Attend meetings and training courses as required
    • Must have the ability to use a computer and have familiarity with Microsoft applications
    • Complete required documentation within the Electronic Health Record (EHR) when interactions occur with the clients
    • Complete the Department of Human Services (DHS) Behavioral Health Critical Incident form or report when a critical incident occurs
    • Collection of rent payments from clients
    • Completes all other duties as assigned
    • Performing on-call responsibilities as assigned

Qualifications:
  • Bachelor’s degree in a social science field; OR associate’s degree in a social science field with work experience in a social service area; OR high school/GED with work experience in the social service field preferred.  Experience in mental health, alcohol and other drugs, adult/geriatric, and the homeless population preferred.
  • Act 33 and Act 34 clearance and FBI background check, if applicable
  • Valid PA Driver’s license
  • Clean driving record

Exceptional Adventures has more than 40 years of experience providing vacation opportunities, social events and monthly dances for individuals with intellectual and developmental disabilities. Our recreational and travel programs are designed to support guests in a wide variety of activities. Organized trips include motor coach, flight, train and cruise packages which are offered year-round.
