Options for Community Living, Inc. is looking for a Housing Placement Assistant to join our team! An ideal candidate should meet the following requirements:
- Bachelor's degree in health or human services and/or RN, LPN, Certified Alcohol Substance Abuse Counselor preferred.
- Case management or case work experience with persons with HIV or AIDS, and/or persons with history of mental illness, homelessness, or chemical dependence, or other populations of persons in need preferred.
- Must have excellent interpersonal skills in order to motivate and assist others.
- Must be able to communicate knowledgeably about the local services and systems of care.
- Bilingual preferred (English/Spanish speaking).
- Valid driver's license, safe driving record, valid auto insurance and access to a vehicle is required.

Salary Range: $41,860/yr. ($23.00/hr.) - $54,600/yr. ($30.00/hr.)
The above salary range represents Options for Community Living's good faith and reasonable estimate of potential compensation that may be offered to a successful applicant for this position at the time of this job posting and may be modified in the future. Schedule Options: (35 hours/week)
- Monday - Friday: 8:00 AM - 4:00 PM
- Monday - Friday: 8:30 AM - 4:30 PM
- Monday - Friday: 9:00 AM - 5:00 PM
Location: In person based out of our Ronkonkoma and Hempstead offices, with field visits required in Nassau & Suffolk Counties.

Responsibilities:
The Housing Placement Assistant will assist in the housing placement and coordination of support services to clients of the ATC Housing and MRT programs by conducting intakes for the programs to fill vacancies as they arise. The HPA will assist with processing housing applications, providing case management, and ensuring access to stable housing. Assists clients in obtaining access to medical care, including accompaniment to appointments, participating in case conferences, assisting in implementing service plan goals and objectives, follow up with clients on their treatment adherence and encourages client self-sufficiency and housing retention. Conducts home visits.
- Conduct outreach to reach homeless individuals and families with HIV/AIDS including agencies which work on behalf of those who need housing.
- Provide housing search and placement services.
- Regularly assist consumers to access entitlements, emergency and transitional housing, and affordable permanent housing placements. Link consumers to permanent housing leads in the community.
- Maintain current list of housing providers, realtors, and landlords in the local community. Maintain regular contact with them to keep updated available housing lists.
- Completes Financial Assistance applications and collects all eligibility documents.
- Completes rent calculations, lease negotiations and HUD inspections on prospective units.
- Conducts housing inspections for new placements and annually.
- Provide case management services, independent living skills training, peer support groups and/or transportation for housing clients.
- Provide transportation services when no other resources are available for consumers.
- Provide referrals, case coordination and follow-up activities as needed to ensure client linkage with case management, counseling, transportation, medical care, child/respite care, partner notification, and other support services, in a professional manner.
- Assist Program Supervisor to gather documentation of units of service for reporting purposes.
- Implement and support service plan goals and monitor objectives in a consistent manner.
- Provide frequent contact and monitoring of progress; document activities concurrently.
- Perform home visits to clients; accompany consumers on medical or housing retention appointments.
- Educate client and family on treatment adherence, health, and human service resources, assist in obtaining services, and follow-up on service delivery on a weekly basis.
- Interact with service providers in a professional manner while advocating for clients' special needs.
- Assist clients and families with problem-solving activities.
- Appropriately intervene in situations requiring immediate attention to ensure safety of clients and family.
- Provide services in a culturally and linguistically competent manner.
- Coach and encourage consumers in self-advocacy and self-management.
- Assist case managers when needed with client interventions.
- Participate in case conferences.
- Assist with maintenance of binder/client files.
- Other appropriate tasks as assigned by management.

$41,860—$54,600 USD
