Primary Accountability
The Human Resources Clerk is responsible for performing various administrative tasks for the Human Resources department, with a primary focus on maintaining internal credentialing and staff appointments to meet the organization's internal compliance requirements.
Description of Primary Responsibilities
Responsible for maintaining employee personnel and credentialing files.
Collects, reviews, scans and organizes credentialing applications and required documentation.
Enters and updates employee data in the HR information system (HRIS).
Responsible for the administrative duties to complete the internal credentialing process for all providers upon hire, appointment, and reappointment, ensuring compliance with State and Federal regulations.
Ensures provider credentialing files are accurate and updated in the provider master database.
Performs or coordinates primary source verification (PSV) of credentials, professional licenses, certifications, registrations, education, training, and work history.
Provides reminder notifications and assists providers with obtaining licensure/DEA.
Prepares and ensures provider files are complete for presentation to the credentialing
Committee and board appointment.
Communicates designated provider information to all internal departments accurately and timely.
Responsible for tracking all provider documentation and expiration dates for licenses,
DEA registrations, certification, and other required documents, and important notifications, to ensure compliance with the regulatory and monitoring requirements for the organization.
Tracks and sends renewal of license reminders, I-9 documentation reminders, department orientation and competency reminders, PPD and CPR renewal notifications for employees and non-employees.
Follows written procedures and meets assigned deadlines.
Responsible for performing various data entry duties.
Follows procedures and meets deadlines for entering PPD, CPR, department orientation, competencies, licenses, benefit enrollment, and other data as assigned.
Responsible for assisting the team with department clerical duties, assigned, including:
Books meeting rooms.
Submits purchase orders.
Processes employee personal information changes.
Prints employee and non-employee badges.
Responds to verification of employment requests.
Assists with audits, as assigned.
Monitors HR inbox and eFax. Distributes correspondence to appropriate HR team members.
Assists employees with HR related questions or refers them to appropriate HR team members.
Responsible for adhering to the Attendance and Absenteeism Policy, recognizing that regular
attendance is considered an essential function of all FHCN positions. Absenteeism is not being at work or failing to attend a paid workshop, training, or event unless the absence is protected by law.
Ability to present to and work at any FHCN location, both at the beginning of a shift or during a
shift, based on business need.
Performs other duties as assigned.

Professional & Technical Knowledge:
Effective June 1, 2026, all individuals hired into the role must:
Possesses work-related skills at a higher level than completion of high school, including formal written and verbal communications skills, computational and computer skills, mathematical, technical or health care related knowledge frequently acquired through completion of a trade school, para-professional, or certificate type program.
Completion of an Associate's Degree program with a recognized major and a minimum cumulative GPA of 2.5 preferred.
If an individual has completed a degree at a higher level than required by the role and had a stronger GPA in that program, they may provide proof of GPA from that degree in lieu of the high school diploma or Associate's degree.
Have a minimum of two years of experience in progressive clerical or administrative responsibility.
Performance of the job duties and responsibilities requires user knowledge of HR technologies, knowledge of databases, and spreadsheets.
