< Position Overview

The Care Manager develops, facilitates, monitors, and communicates individualized care plans in collaboration with members, families, caregivers, physicians, and interdisciplinary providers. Using assessments and UAS information, you will identify risk factors, strengths, challenges, and service needs to ensure members remain safe, healthy, and supported within their communities.

= Key Responsibilities

Review and evaluate member assessments and UAS data for MLTC and MAP populations

Collaborate closely with Primary Care Physicians (PCPs) to discuss member care needs and concerns

Identify risk factors and assign appropriate risk categories

Develop comprehensive care plans, including disaster preparedness plans

Monitor member status monthly through telephonic and face-to-face outreach as needed

Identify clinical issues requiring immediate intervention to prevent:

• Hospitalizations

• Emergency Department visits

• Nursing home admissions

Coordinate services across healthcare networks and community resources

Facilitate smooth transitions of care and discharge planning

Prior authorize service requests using evidence-based guidelines

Partner with Utilization Management (UM) on concurrent and retrospective reviews

Maintain timely documentation and care coordination records

Participate in interdisciplinary care planning meetings

Support grievance resolution and quality improvement initiatives

Identify members eligible for specialty programs

Participate in the rotating on-call schedule

< Qualifications

Required

Active RN License in New York State

Bachelor's Degree (BSN or equivalent)

Minimum 5 years of clinical nursing experience

Experience with:

• Care Coordination

• Utilization Review

• Discharge Planning

Strong computer proficiency and documentation skills

Ability to build collaborative relationships with physicians and interdisciplinary teams

PRI and Screen Certification within 3 months of employment

Preferred

P Leadership experience

P Master's Degree

P Bilingual Spanish

P Case Management Certification
