IBHS Care Coordinator

JOB OVERVIEW:

The Care Coordinator will engage youth, families, and other significant persons involved in the youth's treatment in a collaborative relationship to promote positive outcomes, assess youth and family social determinants of health, and provide supports to address identified physical and behavioral needs. The Care Coordinator assures that the children/families' needs are coordinated among agency departments, outside agencies, and the funding source and that they are connected to any needed resources. The Care Coordinator will be responsible for the day-to-day operations of the school or schools they are assigned to, including responding to school needs, participating in school meetings, processing intakes, and making staff assignments.

RESPONSIBILITIES:

• Provide onsite coordination of case assignment and staffing. Ensure that the day-to-day operations developed by the supervisor are executed seamlessly.
• Gather all necessary documents within the given timeframes for packet submission to CBH, as well as Summer Grant Funding Applications, to ensure consistent authorization of services. Coordinate with the treatment team, family members, and other involved agencies as necessary. This may include visits to the familys home, school, or other community-based locations to obtain and/or complete paperwork.
• Provide BHT coverage on a time-limited basis during times of staff vacancy to ensure program compliance with coverage.
• Document all services provided within 48 hours of service delivery in the EHR. This includes progress notes for BHT coverage, phone calls, emails, and any other correspondence related to a client's services.
• Complete admission paperwork with families as part of the intake process and renew all consents and releases of information annually or as needed.
• Ensure that all members of the IBHS team are aware of packet status, including the Clinical Supervisor.
• Schedule all initial intakes for new referrals and notify all relevant parties.
• Ensure the follow-up of non-clinical recommendations on both psychological and psychiatric evaluations as well as CBH authorization emails, and by the direction of the treatment team.
• Review and update the master case list to show authorization disposition and distribute this information to appropriate team members.
• Participate in weekly team meetings, staff meetings, school meetings, and any additional meetings called by the Program Director, Clinical Supervisor, or Psychologist.
• Participate in and complete all mandatory trainings set forth by stakeholders, including CBH, within the timeframes required.
• Provide coverage on other teams as directed by program leadership.

Administrative Activities

• Ensures compliance with all PHMC's policies and procedures, including confidentiality and acting as a mandated reporter in accordance with the law
• Implements all documentation requirements, ensuring that reports are complete, thorough, timely, and written in the prescribed format; ensures that staff members do the same
• Performs other duties as assigned.

• Familiarity with basic clinical documentation practices, managed care organizations such as CBH and Magellan and the Department of Behavioral Health
• Understanding of theory and implementation of behavior modification interventions
• Ability to quickly form trusting relationships with a child/adolescent population based on an understanding of developmental processes and emotional needs
• Knowledge of crisis management techniques
• Ability to form effective working relationships with diverse individuals, school staff, family members, and stakeholders
• Ability to independently assess and record data in order to report behavioral outcomes and intervention results to the BC, MT, and Psychologist
• Knowledge of, application of, and adherence to CASSP principles
• Ability to effectively and independently communicate treatment objectives and interventions to youth, family, school staff, and other treatment team members
• Ability to administer First Aid and CPR, if necessary

QUALIFICATIONS:

• Must meet one of the following:

• Have a bachelor's degree in psychology, social work, sociology, education, criminal justice, or another human service field, OR

• Have a high school diploma or equivalent AND have completed a 40-hour training covering the RBT Task List (on-the-job training will be provided) AND have at least 2 years of experience in providing behavioral health services

• Preferred: At least 1 year of full-time paid experience working in direct contact with children/adolescents is preferred (or required, depending on degree - see below).
