JOB OVERVIEW:
The Care Coordinator will engage youth, families, and other significant persons involved in the youth's treatment in a collaborative relationship to promote positive outcomes, assess youth and family social determinants of health, and provide supports to address identified physical and behavioral needs. The Care Coordinator assures that the children/families' needs are coordinated among agency departments, outside agencies, and the funding source and that they are connected to any needed resources. The Care Coordinator will be responsible for the day-to-day operations of the school or schools they are assigned to, including responding to school needs, participating in school meetings, processing intakes, and making staff assignments.

QUALIFICATIONS:
Must meet one of the following:
Have a bachelor's degree in psychology, social work, sociology, education, criminal justice, or another human service field, OR
Have a high school diploma or equivalent AND have completed a 40-hour training covering the RBT Task List (on-the-job training will be provided) AND have at least 2 years of experience in providing behavioral health services
Preferred: At least 1 year of full-time paid experience working in direct contact with children/adolescents is preferred (or required, depending on degree - see below).
PAY GRADE: 15
