JOB OVERVIEW:

The ICMS Case Manager provides intensive case management services to families experiencing homelessness or at risk of homelessness. This position assists eligible children (birth to age 18) and their families in obtaining and maintaining transitional and permanent housing while connecting them to medical, educational, social, pre-vocational, rehabilitative, and other community-based resources.

Services are delivered through a Community-Based Scattered Site Model designed to promote family independence, minimize disruption, and support stronger, more sustainable housing outcomes. The ICMS Case Manager works collaboratively with families, housing providers, community partners, and multidisciplinary teams to ensure clients receive comprehensive, coordinated, and culturally responsive services.


ESSENTIAL FUNCTIONS:

  • Maintain accurate, timely, and complete client records within CHAMP and HMIS to ensure data integrity and contract compliance.
  • Develop and maintain effective working relationships with housing property staff, Housing Authorities, service providers, and community partners.
  • Maintain client records and conduct ongoing outreach efforts to engage and re-engage assigned clients as needed.
  • Ensure assigned clients maintain eligibility and compliance with all required housing documentation and program requirements to prevent loss of subsidized housing, including site-based and scattered-site housing.
  • Conduct comprehensive assessments to identify family strengths, needs, barriers, and service goals.
  • Assist families in accessing needed resources, including healthcare, health insurance, housing supports, childcare assistance, employment resources, and other community services through referrals and linkages.
  • Provide individual and group rehabilitation services under applicable HUD, DMH, DPSS, and city-funded contracts.
  • Develop and implement individualized service plans that support improved, maintained, or restored functioning in the home, school, and community.
  • Complete and submit progress notes within required timelines, including documentation within 24 hours, using agency-approved electronic record systems.
  • Provide guidance and support to parents and caregivers regarding child behavior management, family stressors, and strengthening family relationships.
  • Assist families in identifying internal and external resources related to homelessness, domestic violence, substance use challenges, mental health needs, and other barriers affecting stability.
  • Collaborate with therapists, psychiatrists, medical providers, housing staff, and other members of the treatment team to support client goals.
  • Provide interagency and intra-agency consultation, communication, coordination, and referrals.
  • Identify and communicate barriers preventing families from engaging with services and collaborate with the treatment team to address challenges.
  • Promote coordination and alignment among housing, mental health, healthcare, and supportive services.
  • Assist with crisis intervention, grievance processes, and conflict resolution while following safety procedures and housing provider protocols.
  • Maintain client electronic and hard-copy files in an audit-ready condition and ensure compliance with applicable contracts and regulations.
  • Participate in meetings, trainings, learning networks, and professional development activities related to housing, health services, and homelessness.
  • Support agency policies, values, mission, and service delivery standards.
  • Assist with special projects within Housing Services and across the agency as assigned.
  • Perform other related duties as assigned and approved by the supervisor.

QUALIFICATIONS:

Required

  • Bachelor’s degree in social work, psychology, or other mental health related discipline.
  • Minimum of three years of direct service experience in social services, preferably within housing, homelessness, behavioral health, or healthcare settings.
  • Knowledge of Los Angeles Continuum of Care programs, Department of Health Services (DHS), HMIS, and CHAMP systems.

Preferred

  • Bilingual in Spanish.

CONDITIONS OF EMPLOYMENT:

Employee may be asked to participate in cross-training programs or pursue additional education or training when it is determined to be in the best interest of the company by the Chief Operating Officer and Chief Executive Officer.

WORKING CONDITIONS:

  • Work is performed in a normal office setting, client’s homes and the community.
  • Noise level is moderate with occasional loud outbursts.
  • This is a field-based position and requires 80% driving.
  • Will require a personal motor vehicle to frequently travel between offices, housing appointments, and community events.

REASONING ABILITY:

Ability to solve practical problems and deal with a variety of variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

PHYSICAL DEMANDS:

  • Must be able to remain in a stationary position 50% of the time
  • Needs to occasionally move about inside the office to access file cabinets, office machinery, etc.
  • Constantly operates a computer and other office productivity machinery, (i.e., calculator, copy machine and computer printer).
  • Constantly converses with staff and clients
  • Frequently operates a motor vehicle to travel to school or home site
  • Frequently needs to position self in order to effectively interact with children of lesser stature
  • Occasionally needs to transport materials (food, clothing, books, etc.) weighing up to 50 pounds for therapeutic activities
  • Frequently needs to use motor vehicle for business purposes.
