    Our Personal Care Homes provide a welcoming residential environment where Villagers receive individualized support while building independence, meaningful relationships, and a true sense of home. Through compassionate care, person-centered services, and a commitment to community, we help each Villager live a meaningful and fulfilling life.


    Position Purpose

    The Personal Care Home Manager provides leadership and oversight for Annandale Village's Personal Care Home program, ensuring Villagers receive high-quality, person-centered services in a safe, supportive, and engaging home environment.

    This role leads the daily operations of the Personal Care Homes, including employee leadership, staffing, regulatory compliance, quality improvement, and resource management. Working collaboratively with Villagers, families, employees, and interdisciplinary teams, the Personal Care Home Manager promotes a culture of dignity, accountability, continuous improvement, and exceptional service.

    Major Duties and Responsibilities

    Leading Personal Care Home Operations

    • Lead the daily operations of Annandale Village's Personal Care Homes, ensuring safe, high-quality, and person-centered services.
    • Maintain welcoming, well-managed homes that promote Villager dignity, choice, independence, and quality of life.
    • Ensure operations align with organizational policies, state regulations, licensing requirements, and applicable standards.
    • Monitor service quality and identify opportunities to strengthen the Villager experience and operational effectiveness.
    • Serve as the Designated Person in Authority (DPA) in the absence of the Personal Care Home Administrator.

    Supporting Person-Centered Services

    • Ensure services are delivered in accordance with each Villager's Individual Service Plan (ISP), preferences, abilities, and goals.
    • Promote opportunities for Villagers to make choices, build independence, develop meaningful relationships, and participate fully in daily life.
    • Partner with Villagers, families, guardians, and interdisciplinary team members to support individualized needs and positive outcomes.
    • Ensure behavioral, health, safety, and skill-development supports are implemented consistently and respectfully.
    • Participate in interdisciplinary planning and advocate for services that enhance Villager well-being and quality of life.

    Leading and Developing Employees

    • Recruit, onboard, coach, supervise, and evaluate Personal Care Home employees.
    • Foster a culture of accountability, teamwork, professionalism, compassion, and person-centered support.
    • Ensure employees receive required training, maintain necessary competencies, and understand expectations for Villager services.
    • Provide ongoing coaching, feedback, recognition, and performance management to support employee growth and effectiveness.
    • Partner with Human Resources and organizational leadership to support recruitment, retention, and employee engagement.

    Staffing and Operational Effectiveness

    • Ensure appropriate staffing and scheduling to meet Villager needs, regulatory requirements, and operational priorities.
    • Monitor staffing levels, overtime, attendance, shift coverage, and other workforce needs.
    • Respond effectively to staffing challenges and changing Villager needs while maintaining continuity and quality of services.
    • Manage departmental resources and budget responsibly.
    • Oversee supplies, equipment, and facility needs necessary to support effective Personal Care Home operations.

    Quality, Compliance, and Safety

    • Promote compliance with applicable regulations, licensing standards, organizational policies, and quality expectations.
    • Monitor required documentation, incident reporting, medication support processes, and other areas affecting Villager health and safety.
    • Lead and participate in quality assurance and performance improvement activities.
    • Ensure emergency preparedness, infection prevention, and safety practices are implemented consistently.
    • Identify concerns, trends, and opportunities for improvement and ensure appropriate follow-up.

    Collaboration and Communication

    • Build positive relationships with Villagers, families, guardians, employees, and interdisciplinary team members.
    • Partner with Nursing, Social Services, Day Programs, Behavioral Services, Human Resources, and other departments to provide coordinated services.
    • Maintain open, respectful, and timely communication regarding Villager needs, program operations, and areas of concern.
    • Promote collaboration across departments to support continuity of services and a positive Villager experience.
    • Represent the Personal Care Home program professionally within Annandale Village and the broader community.

    Other Responsibilities

    • Provide direct support during emergencies, staffing shortages, or changing Villager needs when necessary.
    • Work a minimum of two evening, overnight, or weekend shifts each month to maintain familiarity with 24/7 Personal Care Home operations and support program needs.
    • Participate in organizational initiatives, meetings, training, and special projects.
    • Perform other duties in the best interest of Annandale Village and the Villagers we serve.

    Required Qualifications

    • Bachelor's degree in Human Services, Healthcare Administration, Business Administration, Nursing, Social Work, or a related field preferred; equivalent combination of education and experience considered.
    • Minimum of three to five years of leadership or management experience in residential services, developmental disabilities, healthcare, assisted living, or a related human services setting.
    • Experience supervising employees in a 24-hour care or support environment preferred.
    • Knowledge of person-centered planning, Personal Care Home services, and regulatory compliance.
    • Experience with electronic health records and workforce scheduling systems preferred.
    • Strong leadership, communication, organizational, and problem-solving skills.

    Employment Requirements

    • Must be at least 21 years of age.
    • Valid Georgia Class C Driver's License with acceptable driving record.
    • Proof of vehicle insurance.
    • Successful completion of GBI/FBI background check and fingerprinting.
    • Successful completion of drug and tuberculosis screening.
    • Ability to obtain and maintain Proxy Caregiver certification.

    Physical Requirements

    • Ability to sit, stand, walk, bend, reach, and perform responsibilities throughout the workday.
    • Ability to provide or assist with hands-on Villager support when necessary.
    • Ability to lift up to 25 pounds independently and up to 75 pounds with assistance.
    • Ability to assist Villagers in and out of vehicles.
    • Ability to perform CPR, First Aid, and emergency response duties as required.
