  • Office location Lewisburg PA (Hybrid Position)
  • Coverage Area: Lewisburg and Wellsboro service areas
  • Travel Required: Regular travel throughout the Lewisburg and Wellsboro regions which may include the following counties:
    • Union, Snyder, Lycoming, Northumberland, Clinton, Columbia, Montour, Tioga, Bradford, Sullivan, Susquehanna, and Potter Counties
    • Must be willing to travel to each offices service area at least twice per week
  • Schedule: Full time, day hours,35 hours per week (may expand to 40 hours as caseloads grow)
  • On-Call Rotation: Participation required

CRI is currently currently seeking an experienced and motivated IDD Program Specialist to support individuals receiving services through the Office of Developmental Programs. This position is responsible for program oversight activities includingmonitoring services and documentation, completing reporting requirements, supporting staff training, ensuring regulatory compliance, and promoting quality services for participants throughout the Lewisburg and Wellsboro regions.

This is a office-based position andhybrid work-from-home. The successful candidate will be expected to work from the Lewisburg office at least one day per week plus outreach for the the Lewisburg office one day, and work from theWellsboro office at least one day per week and provide outreach for the Wellsboro office one day per week. Generally Mondays and Fridays, while also traveling throughout the service areas to meet with care planning teams, participants, community partners for outreach and referral sources.

Job responsibilities include, but are not limited to:

  • Intake and enrollment of new participants
  • Monitoring participant services and documentation to ensure quality and compliance
  • Reviewing participant logs, service notes, and documentation regularly for concerns and accuracy
  • Trackingparticipantprogress towards ISP goals and identifying service needs
  • Prepare and submit monthly, quarterly, annual, and biennial progress reports
  • Attending participant meetings, including ISP meetings, critical revision meetings, behavioral support plan meetings, and other care planning meetings as needed
  • Meeting with Administrative Entities, Supports Coordination Organizations, referral sources, and community partners
  • Conducting participant/client check-ins and quarterly shift monitoring
  • Assist in training/mentoring of Direct Support Professionals on IDD program requirements and participant ISP plan review prior to DSP serving that participant
  • Assisting supervisors with accountability and oversight ofdirect support professional documentation performance
  • Reporting observations and concerns regarding DSP performance to supervisors
  • Assist with preparation for state audits and participating in audit activities
  • Prepare and submit quarterly progress reports for each IDD participant.
  • Conduct Certified Investigations and reports
  • Review participant logs monthly or more frequently as necessary.
  • Participate in supervisory on-call rotation
  • Participate in outreach and relationship-building opportunities within the community

Requirements:

  • Bachelor's Degree in related field preferred
  • Experience working within IDD or human services programs preferred
  • Strong organizational, communication, and documentation skills
  • Ability to work independently and collaboratively across multiple teams locations
  • Ability to pass background checks
  • Satisfactory driver's record check, valid driver's license, and reliable vehicle
  • Obtain and maintain Office of Developmental Program Certified Investigator certification
  • Be at least 18 years of age and legally able to work in the United States
  • Flexibility toattend program-related meetings outside normal business hours, including occasional evenings or weekends as needed
