The Provider performs in‑person, no‑cost annual preventive Health Risk Assessments (HRAs) for Medicare, Medicaid, and Affordable Care Act members. Visits occur in members’ homes and follow health plan and regulatory expectations. This role supports improved care quality, risk adjustment accuracy, and member experience.
Providers must be credentialed with Excellus, NYS Medicare, and Medicaid to avoid onboarding delays.
Conduct comprehensive in‑home health risk assessments for eligible members.
Deliver high‑quality evaluations aligned with clinical standards and health plan requirements.
Provide 15 completed visits per month (minimum of 15 appointment slots).
Document and submit chart notes within 24 hours of each visit.
Respond to charting queries within 48 hours.
Maintain strong communication with clinical leadership and support teams.
Attend required training sessions and meetings.
Safely enter and work within members’ personal living environments.
Lift up to 20 lbs and bend/kneel for extended periods as needed.
Flexible scheduling: daytime and evening availability.
Appointment duration: 60–75 minutes.
Compensation: Market rate per completed assessment.
Mileage reimbursement: After the first 30 miles driven daily, reimbursed at current IRS rate.
Proficiency with electronic medical records and strong computer skills.
Understanding of CMS and ACA risk adjustment models.
Ability to document clinical findings accurately and thoroughly.
Strong verbal and written communication skills.
Ability to maintain confidentiality and comply with HIPAA standards.
Sound clinical judgment and adherence to policies and procedures.
Adaptability in dynamic environments and changing member needs.
Positive, collaborative approach with members and clinical teams.
Ability to proactively identify and resolve member or provider issues.
Master’s or Doctorate degree from an accredited Nurse Practitioner program (FNP, AGNP, ANP) or Master’s degree from an accredited Physician Assistant program.
Home health experience within the last 5 years (required).
At least 1 year of in‑home assessment experience (highly preferred).
Active RN and NP or PA license in the state of practice.
Board certification (ANCC, AANP, or NCCPA).
Active Medicaid ID for the state.
Active Medicare PTAN.
Current BLS certification.
Valid, unencumbered driver’s license.
Proof of adequate auto insurance for the state of practice.
