Schedule: create your own flexible schedule, Mon - Sat, 7am - 7pm
Visits: 15 per month minimum
Travel: Required within a 50-mile radius from your residence
Mileage reimbursement: after the first 30 miles driven per day (IRS rate)
Complete required self-paced and instructor-led training within allotted time
Provide at least 15 appointment slots per month
Deliver high-quality assessments that meet member satisfaction and health plan standards
Document and submit chart notes within 24 hours of each visit
Address charting queries within 48 hours
Master’s or Doctorate degree from an accredited NP program (FNP, AGNP, ANP)
Home health experience required within the last 5 years
1 year of in-home assessment experience highly preferred
Current RN and NP or PA licensure in state of practice
Board certification (ANCC, AANP, or NCCPA)
Current Medicaid ID number for state
Current Medicare PTAN number
Current BLS certification
