Location: Syracuse, NY
Description: We are seeking dedicated Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our team in a per diem, temp-to-permanent role. This position involves conducting in-person, no-cost annual preventative health risk assessments for Medicare, Medicaid, and Affordable Care Act members. Assessments are performed in members’ personal living spaces in accordance with health plan expectations.

Assessments are performed in members’ personal living spaces in accordance with health plan expectations.
Schedule: Create your own Schedule, Hours of Operation are 7am-7pm Mon-Sat
Visits: 15 per month minimum
Travel: Required within a 50-mile radius from your residence
Mileage reimbursement after the first 30 miles driven per day (IRS rate)
Essential Functions
Complete required self-paced and instructor-led training within allotted time
Provide at least 30 appointment slots per month
Deliver high-quality assessments that meet member satisfaction and health plan standards
Document and submit chart notes within 24 hours of each visit
Address charting queries within 48 hours
Education & Experience
Master’s or Doctorate degree from an accredited NP program (FNP, AGNP, ANP)
Home health experience required within the last 5 years
1 year of in-home assessment experience highly preferred
Certification & Licensure
Current RN and NP or PA licensure in state of practice
Board certification (ANCC, AANP, or NCCPA)
Current Medicaid ID number for state
Current Medicare PTAN number
Current BLS certification
