Location: Within a 50‑minute travel radius in assigned county Schedule: Flexible days/evenings; 60–75‑minute appointment windows Reports to: Manager of Care Delivery Compensation: Market rate per completed assessment; mileage reimbursed after first 30 miles/day at current IRS rate Workload: 25–30 visits per month (minimum)
You will conduct in‑person, no‑cost annual preventive health risk assessments for Medicare, Medicaid, and ACA members. Visits occur in the member’s home and follow health plan quality and documentation standards. This role is ideal for clinicians who value autonomy, flexibility, and meaningful one‑on‑one patient interactions.
Perform comprehensive in‑home health risk assessments for eligible members.
Deliver high‑quality, patient‑centered evaluations aligned with health plan expectations.
Provide a minimum of 25–30 appointment slots per month.
Complete and submit documentation within 24 hours of each visit.
Respond to charting queries within 48 hours.
Maintain clear and timely communication with clinical leadership.
Participate in required training sessions and periodic meetings.
Safely enter and work within members’ personal living environments.
Lift up to 20 lbs and bend/kneel as needed during assessments.
Master’s or Doctorate degree from an accredited NP program (FNP, AGNP, ANP) or Master’s degree from an accredited PA program.
Current, active RN and NP license or PA license in the state of practice.
Board certification (ANCC, AANP, or NCCPA).
Valid Medicaid ID and Medicare PTAN.
Current BLS certification.
Valid driver’s license and proof of required auto insurance.
Home health experience within the last 5 years.
1+ year of in‑home assessment experience.
Strong proficiency with electronic medical records and digital documentation.
Understanding of CMS and ACA risk‑adjustment models.
Ability to make sound clinical decisions while following established protocols.
Strong written and verbal communication skills.
Ability to maintain confidentiality and comply with HIPAA requirements.
Adaptability, professionalism, and a collaborative mindset.
Accurate and thorough clinical documentation.
Strong computer and EMR skills.
Ability to anticipate and resolve member, provider, and health plan concerns.
Positive, patient‑focused communication style.
Flexibility and comfort working in varied home environments.
