About the Role:
All Care Companions LLC is looking for a compassionate and dedicated In Home and Community Care Support Individual to join their team in Eagan, MN. Responsibilities:

• Provide direct, hands-on personal care and daily living assistance to clients in home and community settings
• Support clients with activities of daily living (ADLs) including bathing, grooming, dressing, and meal preparation
• Accompany and assist clients with community outings, appointments, and social activities
• Monitor and document client health, behavior, and progress, reporting changes to supervisors
• Implement individualized care plans tailored to each client's unique needs and goals
• Foster a safe, supportive, and dignified environment for all clients
• Build trusting relationships with clients, families, and care team members

Requirements:
Schedule: 8:00am-2:00pm M-F

• Family prefers female caregiver and dog friendly person 
• Prior experience in home care, personal care assistance (PCA), direct support, or a related caregiving role preferred
• Genuine compassion, patience, and dedication to supporting individuals with diverse needs
• Ability to pass a background check as required by Minnesota state regulations
• Valid driver's license and reliable transportation preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a collaborative care team
