We’re a nurse-led, advocacy-driven team where caregivers are an important part of the care team. You’ll have the support of Nurse Case Managers while building a consistent relationship with your Client.
Location: Naperville, IL
Schedule: 24/7 care provided by a consistent team of 4 female Caregivers, working 12-hour shifts
Pay: Starting at $17/hour
Overtime: Time-and-a-half after 40 hours/week
This Client requires a Care Professional who is comfortable providing care for someone with advanced dementia and understands the patience, consistency, and safety awareness this type of care requires.
Provide compassionate, one-on-one care in a private home
Assist with ADLs, including bathing, toileting, dressing, grooming, meals, and mobility
Provide hands-on support for a Client with advanced dementia
Maintain a calm and structured environment
Monitor for changes in behavior, condition, or care needs and communicate concerns to the care team
Work collaboratively with the Client’s family and Nurse Case Managers
Follow the established care plan and safety protocols
Build a trusting, consistent relationship with the Client
Consistent Team Environment – Work as part of a dedicated team of 4 caregivers
12-Hour Shifts – Longer shifts provide consistency for both the Client and Care Professional
Nurse Support – You’re backed by experienced Nurse Case Managers
Consistent Client – This is an ongoing 24/7 care case
Meaningful Relationships – Build trust and familiarity with your Client over time
Respect & Teamwork – You’re treated as an important member of the care team
Overtime Opportunities – Time-and-a-half after 40 hours/week
We’re looking for a Care Professional or CNA who:
Has demonstrated experience caring for individuals with advanced dementia
Is comfortable with behaviors and care needs associated with advanced dementia
Has experience providing hands-on personal care
Is patient, dependable, compassionate, and attentive
Communicates well with families and the care team
Can work effectively as part of a small, consistent caregiving team
Understands the importance of consistency and routine in dementia care
Is committed to providing safe, respectful, person-centered care
Advanced dementia experience is required for this opportunity.
