About This Role
We are seeking a compassionate, reliable, and fluent Spanish-speaking Care Partner to support in-home dialysis for a patient who currently receives treatments in a clinic and will transition to home dialysis. This is a one-on-one, consistent assignment (not an agency rotation), allowing you to build trust, rapport, and become an integral part of the patient's ongoing care routine.
Comprehensive training will be provided, including patient-specific dialysis procedures, home setup guidance, and continued support from our clinical team, ensuring you feel confident and well-supported in this role.

Primary Goal
Provide safe, consistent, and high-quality dialysis support in the home while reducing physical and logistical strain on the patient and family.
Key Responsibilities
- Assist with in-home dialysis treatment setup (NxStage or similar systems)
- Maintain a clean, organized, and safe treatment environment
- Support the patient in staying on schedule with dialysis treatments
- Provide companionship and emotional reassurance
- Assist with light Activities of Daily Living (ADLs) as needed
- Monitor patient safety and report concerns to the coordinating nurse
- Maintain simple documentation and shift notes
Qualifications
- Minimum 1 year of dialysis experience (home or clinic)
- Familiarity with home dialysis systems preferred (NxStage a plus)
- Fluent in Spanish (required)
- Open to in-home dialysis care and completing provided training
- Calm, patient, and dependable—especially during care transitions
- Strong communication skills and ability to collaborate with a clinical team
- Must pass a Herewith background check
