Schedule: Monday to Friday, 8:30 am to 5:00 pm
Salary: $30.56 (hourly) Non-Exempt

Job Description:
We are seeking a compassionate and motivated In-Home Outreach Counselor to provide direct outreach, counseling, and case management services to individuals and families in the community. The ideal candidate will engage residents through in-home outreach and coordinate services that promote stability, independence, and overall well-being.
Duties:
- Conduct in-home outreach services, including distributing literature, door-to-door canvassing, and engaging residents through phone and in-person contact.
- Target and engage existing community groups and activities within residential developments.
- Provide in-home counseling services to clients and their families in accordance with the appropriate level of care.
- Coordinate and provide support services that assist clients with independent living skills.
- Deliver comprehensive case management services, including linking clients to auxiliary services and assisting with access to community resources.
- Assess client and family needs related to social, emotional, financial, and medical challenges, and coordinate appropriate service delivery.
- Collaborate and maintain communication with referral sources such as the Department of Children and Family Services (DCFS) and the Department of Probation.
- Maintain positive and professional working relationships with community partners, linkage agencies, and referral organizations.
- Stay informed of available community programs, services offered, and eligibility requirements.
- Maintain accurate and timely case documentation in compliance with agency policies and all applicable federal, state, county, and local regulations.

Job Requirements:
- Bachelors degree in Social Work, Psychology, Counseling, or a related field required; Masters degree preferred.
- Bachelors degree candidates must have a minimum of two (2) years of direct service experience in social services or a related field (DCFS approval required).
- Experience working with high-risk families and children, under-served populations, and/or substance abuse issues preferred.
- Demonstrated therapeutic, advocacy, and case management skills through education, training, or professional experience.
- Strong verbal and written communication skills, with the ability to present information clearly and effectively.
- Proficiency in basic computer skills, including word processing software.
- Ability to exercise sound judgment, analyze situations, and make independent decisions.
- Commitment to teamwork and agency values and philosophies.
- Ability to adapt to changing program needs and acquire new skills as needed.
- Culturally responsive and respectful approach when working with diverse communities and clients.
- Valid California drivers license, acceptable driving record, car insurance, car registration, and access to reliable transportation required.
