Physical Therapist (Home Health) – Full-Time | 32–40 Hours Guaranteed

Schedule: Monday–Friday, 8:00 AM–4:30 PM Location: York County & Western Adams County Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last  On‑Call/Weekend Requirements: None

About the Role

We are seeking a dedicated and self‑driven Physical Therapist to serve as the primary PT for our home health agency. This role offers a weekly hour guarantee of 32–40 hours, with typical weeks trending closer to 40. You’ll have the autonomy to manage your own caseload while providing high‑quality, patient‑centered care in a supportive environment.

Key Responsibilities

• Complete Start of Care (SOC) assessments, evaluations, reassessments, and discharges

• Develop and maintain individualized Plans of Care (POCs)

• Manage and prioritize your own caseload

• Supervise and collaborate with Physical Therapist Assistants (PTAs)

• Schedule and communicate with patients daily

• Ensure documentation meets agency and regulatory standards

• Meet productivity expectations of 30 points per week

Required Qualifications

• Physical Therapy degree from an accredited program

• Active state licensure as a Physical Therapist

• Home health experience preferred, but not required

Preferred Skills (Nice-to-Haves)

• Experience with Homecare Homebase (HCHB)

• Familiarity with OASIS documentation

• Strong communication, organization, and time‑management abilities
