Registered Nurse (RN) – Home Health 

Location: Gwinnett, DeKalb, and Fulton Counties 

Schedule: Monday–Friday, 8:00 AM–4:30 PM 

Weekly Hours: Guaranteed 32 hours 

On‑Call Requirement: Yes, one weekend per month 

Mileage Reimbursement: Yes, from first patient to last patient

Rate: $50.00 per hour

This is a contract assignment likely to go perm but no guarantees.

Position Overview

We are seeking an experienced and highly skilled Home Health Registered Nurse (RN) to join our team. This role is responsible for delivering high‑quality skilled nursing care to patients in their homes while meeting weekly productivity expectations.

Key Responsibilities

• Perform skilled nursing home health visits in assigned territories

• Complete a minimum of 30 points per week

• Conduct comprehensive patient assessments and develop individualized care plans

• Document all care accurately and efficiently in accordance with agency standards

• Collaborate with interdisciplinary team members to ensure optimal patient outcomes

• Participate in rotating on‑call schedule (once monthly)

Required Qualifications

• Active RN license in the state of Georgia

• OASIS experience

• HCHB (Homecare Homebase) experience

• Reliable transportation and ability to travel throughout Gwinnett, DeKalb, and Fulton counties

Preferred Skills & Experience

• Previous home health nursing experience

• Strong clinical judgment and assessment abilities
