We are seeking an experienced Travel Registered Nurse (RN) to provide high‑quality skilled nursing care in a home health and hospice setting. This role supports patients across a broad geographic area and requires strong clinical judgment, autonomy, and excellent communication skills.
Deliver skilled nursing care to patients in their homes in accordance with the plan of care
Perform comprehensive assessments and ongoing monitoring of patient conditions
Collaborate with interdisciplinary team members to support patient goals
Document all care accurately and efficiently within required systems
Participate in on‑call rotation and occasional weekend coverage
Travel throughout the assigned 60‑mile radius territory
Active Registered Nurse (RN) license
Previous clinical nursing experience
Valid driver’s license and reliable transportation
HomeCare HomeBase (HCHB) EMR experience
Prior home health or hospice nursing experience
Strong time‑management and independent decision‑making skills
Productivity Expectation: 30 points
