Nurse Practitioner – In-Home Wound Care
Location: Greater Nashville, TN
Compensation: Base salary + equity opportunity
Start Date: Approximately 3 weeks after submission

We are seeking an experienced Nurse Practitioner to join a growing in-home wound care program serving the Greater Nashville area.

This is a unique opportunity for a provider who wants more than a traditional clinical role. We are looking for someone who is excited to help build a program, shape how care is delivered, and grow into a leadership position over time.

In the early stages, this role will be highly hands-on clinically. You will be seeing patients directly while also helping develop the workflows, structure, and care model that will support long-term growth. As the program expands and additional providers are added, the goal is for this individual to take on increasing leadership and management responsibility.

Territory

The primary service area will be between Dickson and Nashville, with the main focus in the Nashville metro area.

During the early phase of growth, this provider must also be willing to travel to Clarksville and West Tennessee markets such as Paris and Martin until additional providers are hired in those areas.

What you'll be doing

  • Provide in-home wound care visits
  • Perform wound assessments, treatment planning, and ongoing care management
  • Perform debridement if within scope
  • Educate patients and caregivers
  • Complete documentation in a timely and accurate manner
  • Help design and refine the workflows, protocols, and systems that will support the program as it grows
  • Collaborate with referring providers and other care team members
  • Provide remote wound care consultation support during nights and weekends, as needed

What we're looking for

Required

  • Active, unrestricted Tennessee Nurse Practitioner license
  • Wound care experience is required
  • Willingness to travel throughout the assigned territory, including initial support in Clarksville and West Tennessee
  • Ability to work independently and confidently in a field-based setting

Strongly preferred

  • Wound Care Certification (WCC, CWCN, or equivalent)
  • Prior in-home wound care experience
  • Experience working autonomously in a mobile or community-based setting
