Pay Rate: Based on Experience

Position Summary

The Independent Support Coordinator Mentor oversees and coordinates the implementation of Person-Centered Planning in the lives of people supported by Community Living Services of Oakland County and assures that the Oakland Community Health Network’s (OCHN) expectations and standards are met.  The Independent Support Coordinator Mentor plays a key role in the recruitment, training, and monitoring of Independent Supports Coordinators.  The Independent Support Coordinator Mentor’s responsibilities include the mentoring of Independent Support Coordinators in Person-Centered Planning, the authorization process, community outreach, obtaining and utilizing stakeholder input, assuring quality services and satisfaction with services while keeping the people supported by CLS and their needs the primary focus of all activities.

This position also functions as the Support Coordination Manager.  The Supports Coordination Manager is responsible for direct supervision and oversight of the CLS – Oakland County internal Support Coordinators.  The Supports Coordination Manager serves as a trainer, supervisor, mentor, and advocate for Supports Coordinators providing services to individuals with developmental disabilities.  The essential function of the position is to facilitate the effective and efficient planning and implementation of the principles of Person-Centered Planning and Self-Determination, the Michigan Mental Health Code, CLS guiding principles, code of ethics, and professional code of ethics.

Key Responsibilities

Customer Service & Outreach

  • Address stakeholder concerns and ensure timely follow-up.
  • Represent CLS at community events, schools, and conferences to promote Self-Determination principles.
  • Foster positive relationships and deliver exceptional customer service.

Person-Centered Planning & Budget Oversight

  • Monitor planning processes and ensure accurate, timely IPOS documentation.
  • Assist with individual budget development and Medicaid compliance.
  • Review and track budgets, resolve discrepancies, and support utilization management.

Leadership & Mentorship

  • Provide guidance and training to Independent Support Coordinators.
  • Facilitate monthly training sessions and maintain a qualified ISC network.
  • Oversee documentation compliance and reinforce organizational standards.

Quality Improvement & Compliance

  • Develop and monitor systems for service quality and regulatory compliance.
  • Participate in audits, corrective action planning, and policy development.
  • Represent CLS in hearings and grievance processes as needed.

Qualifications

  • Bachelor’s degree in Human Services (required).
  • Minimum 1 year experience with intellectual/developmental disabilities; QIDP status required.
  • Supervisory experience preferred.
  • Strong knowledge of Person-Centered Planning, Self-Determination, and ISC models.
  • Excellent communication, organizational, and computer skills.

  • Extensive travel: valid driver’s license and insurance required.
  • Ability to work flexible hours and maintain accurate documentation.
  • Completion of mandatory training and ongoing professional development.
