Bargaining Unit: Outpatient Clinics - EAP

Rate of Pay: $72.79/hour + DOE (Base Pay of $64.71/hour includes 12.5% per diem premium)

Summary
Performs the nursing process through assessment, planning, implementation and evaluation of safe therapeutic care to patients in the Multispecialty Clinics by performing the following duties.
Essential Duties and Responsibilities
  • Assesses the physical and psychosocial needs of patients.
  • Implements and evaluates individual plan of care.
  • Evaluates the effectiveness of interventions.
  • Identifies and provides education to patient and/or significant other.
  • Coordinates resources for complete and effective patient care, primarily focused on electronic health record in basket management.
  • Communicates effectively using Electronic Health Record (EHR) messaging platforms.
  • Participates in departmental growth and development.
  • Rotates and covers in-basket management between departments as assigned/required.
  • Pursues professional growth.
  • Follows patient care standards.
  • Explains medications, in-basket concerns and solutions to patient to gain cooperation, understanding, and allay apprehension.
  • Maintains awareness of comfort and safety needs of patient.
  • Records significant conditions and reactions and notifies provider of patient’s condition.
  • Works effectively in a remote position, finds additional tasks as time allows (some in office hours are required).
  • Responds to life saving situations based upon nursing standards, policies, procedures, and protocol.
  • Initiates patient education according to individualized needs of the patient, as prescribed by provider and/or hospital policy including patient and family instruction.
  • Possesses and develops subject matter knowledge and expertise in assigned departments through partnership and mentoring with physicians and Advance Practice Providers (APP).
  • Reviews medical history, clinical information, and document findings accurately in electronic health record.
  • Assists providers with medication management, refills of medication, MyChart message review in the electronic medical record system.
  • Assists with development and implementation of medication refill protocols.
  • Telephone triage for symptom management.
  • Assists in the development and maintenance of chronic disease registries.
  • Communicates clearly.
  • Establishes and maintains effective, professional working relationships with patients, medical staff, co-workers, and the public.
  • Works with MSC Manager/Director on action plans for process improvement and Quality reporting.
  • Demonstrates System Values in performance and behavior.
  • Complies with System policies and procedures.
  • Other duties as may be assigned.

Supervisory ResponsibilitiesNo supervisory responsibilities

Minimum Education/ExperienceAssociate's Degree Certificate of completion from college or vocational school from a National League of Nursing (NLN), Commission on Collegiate Nursing Education (CCNE) or equivalent accredited school of nursing. The nurse shall have completed a nursing education program that included clinical experiences with actual patients under the supervision of qualified faculty.

1-2 years relevant experience

Required Licenses/CertificationsCalifornia Registered Nurse (RN)
  • Upon hire
Nevada Registered Nurse (RN)
  • Within one (1 ) month of hire into job classification
Basic Life Support (BLS) for Healthcare Providers
  • Upon hire
Other Experience/QualificationsRequired:
  • Possesses effective communication, interpersonal, and teamwork skills, specifically adept with communication using EHR messaging platforms.
  • Adept and comfortable utilizing standardized nursing protocols.
  • Proficiency in the utilization of Epic and other hospital related electronic health record systems.
Preferred:
  • Prior in-basket management and/or triage assessment skills
