About the Role:
Descanso Home Health Services is looking for a compassionate and skilled Home Health Registered Nurse (RN) with infusion expertise to join our dedicated team in Cypress, CA. Responsibilities:

• Administer IV infusion therapies including antibiotics, chemotherapy, TPN, and other prescribed infusion treatments in the home setting
• Conduct comprehensive patient assessments and develop individualized care plans
• Monitor and document patient responses to infusion therapy and report changes to the physician
• Educate patients and caregivers on medication management, infusion care, and safety protocols
• Maintain and troubleshoot infusion pumps, PICC lines, central venous catheters, and other vascular access devices
• Coordinate care with physicians, pharmacists, and interdisciplinary team members
• Ensure accurate and timely clinical documentation in compliance with home health regulations

Requirements:

• Active California Registered Nurse (RN) license in good standing
• Minimum 2+ years of infusion therapy or IV experience required
• Oasis Charting experience
• Home health experience preferred but not required
• PICC line and central line management experience strongly preferred
• Current CPR certification
• Valid California driver's license and reliable transportation
• Strong clinical assessment, critical thinking, and patient communication skills
• Self-motivated, organized, and able to work independently in a field-based environment
