Infusion Pharmacist Position Details
Facility: BayCare HomeCare Location: Largo, FL Status: PRN - as needed, exempt Shifts: 8:00 AM - 5:00 PM
Infusion Pharmacist Responsibilities:
• Reviews and verifies all prescription medication orders received from the prescriber or providers agent and evaluates appropriateness and completeness of prescribed therapies in a home setting. • Assesses the patient at the initiation of IV drug therapy, develops a patient care plan, and drug monitoring regimen, and monitors the patient according to the care plan throughout the course of therapy. • Ensures drug-related necessary supplies and all patient collateral education materials are accurately provided to our patients. • Facilitates communication with the patient care giver, physician, provider, home health nursing staff, and other members of the health care team regarding patient status, response to therapy, and clinical objectives to ensure optimal clinical outcomes. • When required, must be able to compound intravenous and specialty pharmaceuticals products using aseptic techniques according to established Florida Board of Pharmacy, federal agencies, USP<797 & 800> sterile guidelines, and company policy. • Must be knowledgeable of the proper standards, use, and handling of both sterile and hazardous medications. • Ensures compliance with all FDA, DEA, Florida Board of Pharmacy, OSHA, and other regulatory and accrediting entities. Follows all company and department policies and procedures. • Provides direct and indirect oversight of pharmacy technician and pharmacy support staff. • Duties may also include but not limited to: • Special assignments • Projects • Process validation • Competency assessment
Certifications and Licensure Required:
• Licensed Pharmacist - State of FL • Preferred: Board Certification BCSCP
Qualifications Required:
• Bachelors in pharmacy • PharmD preferred • Pharmacist license in good standing in the state of Florida • 1 year of practicing Pharmacist experience working in home infusion, hospital, or related healthcare environment • Knowledgeable in various infusion and specialty related therapies and ability to work through the complexity of therapy-related orders and the ability to fill medication orders accurately and in a timely manner • Willing and able to take calls after hours, weekends, and holidays as scheduled • Experience working collaboratively with an intradisciplinary team and independently with minimal supervision • Preferred: • Strong clinical experience with IV Antibiotics, Inotropes, Parental & Enteral Nutrition, Immunoglobulins, and/or Biologics • Home Infusion experience and/or specialty pharmacy experience • Sterile Compounding experience
