Job Description
Primarily assigned to the Emergency Department, the Medication History Technician is tasked with reviewing and updating patients' medication lists within the electronic medical record to ensure the utmost accuracy for individuals being admitted to the hospital. This process entails a thorough review of office visit documentation, recent hospital discharge summaries, and pharmacy records, in addition to conducting interviews with patients and their family members to confirm all current medications.
The technician is responsible for verifying the names, dosages, and administration instructions of all medications, including prescription drugs, over-the-counter supplements, sample medications provided by physicians, and medications taken on an as-needed basis. When discrepancies arise or information is incomplete, the technician may contact pharmacies or healthcare providers to obtain clarification or additional details.
Upon completion of the verification process, the technician updates the medication list in the electronic medical record, documents their findings in a progress note, and informs the provider that the medication history has been accurately updated.
Education & Experience Requirements
High school diploma or GED required
Valid Pharmacy Technician license from the State of Michigan
Minimum one year of experience as a Hospital Pharmacy Technician or Retail Pharmacy Technician
(Retail experience applies only to medication history positions)
Ability to accurately maintain records and perform calculations
Ability to safely handle and manipulate cytotoxic and other special‑handling medications, when applicable
Ability to work aseptically in a vertical flow hood environment, when required
Ability to maintain Investigational Drug Research Records and validate doses using investigational drug protocols
Ability to lift, carry, and stock pharmacy inventory and transport medications to nursing units
Ability to use a computer keyboard and type approximately 25 WPM
Ability to read, interpret, and apply literature related to medications requiring special handling
Ability to assist in developing or modifying departmental protocols under pharmacist supervision
