Responsibilities:

• Conduct comprehensive assessments of patients to determine care needs.
• Collaborate with healthcare providers to develop and implement care plans.
• Monitor patient progress and adjust care plans as necessary.
• Facilitate communication between patients, families, and healthcare teams.
• Ensure compliance with regulatory and accreditation standards.
• Educate patients and families about treatment options and resources.
• Document all case management activities in accordance with policies.
• Participate in quality improvement initiatives and team meetings.

Requirements:

• Active RN license in California with a minimum of 2 years nursing experience.
• Experience in case management or utilization management preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively in a fast-paced environment.
• Knowledge of healthcare regulations and managed care principles.
• Certification in Case Management (CCM) is a plus.
• Proficient in electronic health records and documentation practices.
