Job Description:
The School-Based Transition Specialist supports students with special needs transitioning from Ben Bronz Academy into another school, college, career, or vocational program. Working with staff, families, employers, and community partners, they provide personalized guidance to empower students to develop independence, daily living skills, and post-secondary success.
Essential Job Functions & Responsibilities
- Individualized Job Coaching: Provide 1-on-1 or small group coaching tailored to unique abilities, interests, and vocational goals.
- Vocational Assessments: Conduct assessments to identify strengths, skills, and growth areas; use results to guide individualized vocational plans.
- Job Development & Placement: Collaborate with local employers and businesses to identify suitable job opportunities, internships, work experiences, and training programs.
- On-the-Job Training: Provide on-site workplace training and support to help students master tasks, foster independence, and build confidence.
- Accommodations & Assistive Technology: Assist students in accessing required workplace accommodations and assistive technology.
- Task Analysis: Break down job tasks into manageable steps, teaching students how to perform them effectively and efficiently.
- Social & Communication Skills: Coach students on social skills, communication techniques, and appropriate behavior for professional interactions.
- Problem-Solving & Conflict Resolution: Teach strategies to address workplace challenges and resolve conflicts.
- Progress Monitoring & Reporting: Regularly assess performance, track progress, maintain detailed documentation, and prepare reports for students, parents, and school staff.
- Collaboration: Work seamlessly with teachers, families, and community partners to ensure student needs are met both in and out of school.
- Parent Engagement: Maintain open communication with parents, providing progress updates and addressing concerns.
Skills & Competencies
- Excellent interpersonal and communication skills (verbal and written).
- Deep empathy and understanding of the unique needs of students with disabilities.
- Knowledge of vocational training techniques and workplace accommodations.
- Strong organizational skills to manage multiple student plans and progress tracking.
- Flexibility and adaptability to respond to varying student needs.
- Proficiency in data collection, reporting, and analyzing assessment results to triangulate data and draft student goals.
- Mandatory License: Valid CT driver's license for at least 3 consecutive years of driving experience with either an "A" or "V" endorsement.
Work Experience Requirements
- Experience working in a school environment preferred.
- Ability to implement school policies and rules governing student life and conduct.

- Strict adherence to confidentiality regarding personal student and family information.
- Ability to work effectively as a team member.
- Uphold the ethics of the teaching profession (CT Code of Professional Responsibility for Teachers).
Education Requirements
- At least an Associate's Degree or higher.
- Preferred experience in job coaching, vocational training, or working with individuals with special needs.
- Knowledge of federal and state laws related to special education and vocational rehabilitation, including IDEA (Individuals with Disabilities Education Act) and the Rehabilitation Act.
