Summary
We are seeking a detail-oriented and patient-focused Home Infusion Intake Representative to join our team. In this key role, you will manage the intake process for patients beginning high-cost IVIG infusion therapy. You'll serve as a liaison between healthcare providers, insurance payers, internal pharmacy teams, and patients to ensure a smooth and timely transition of care. Your coordination skills, compassion, and industry knowledge will directly impact patient outcomes and satisfaction.
What You'll Do - Patient Intake & Assessment: Review clinical documentation to determine eligibility for home infusion services, for high-cost IVIG therapies, while assessing social and home environments for therapy suitability.
- Insurance Verification: Confirm insurance benefits and obtain authorizations for high-cost IVIG services in collaboration with the billing and reimbursement teams.
- Care Coordination: Facilitate communication between physicians, nurses, discharge planners, and pharmacy staff to streamline the start of care for home infusion therapy.
- Patient Education: Provide clear, supportive education to patients and families about therapy schedules, medication delivery, and what to expect with home-based infusion.
- Scheduling: Manage and maintain refill schedules for IVIG patients to ensure uninterrupted therapy.
- Documentation: Keep accurate, up-to-date records in the EMR system, logging communication and tracking each patient's intake progress.
- Collaboration: Work closely with pharmacy operations and clinical teams to ensure the timely and accurate delivery of medications, supplies, and nursing coordination.
- Compliance & Quality Assurance: Ensure compliance with all healthcare regulations and internal protocols while supporting initiatives that improve the overall intake experience.
What You'll Bring- Intake experience with high-cost IVIG therapy coordination in a home infusion setting is required.
- Knowledge of payer requirements and billing practices for infusion therapies.
- Excellent communication and interpersonal skills with a strong commitment to patient care.
- Strong organizational skills and the ability to manage multiple patient transitions concurrently.
- Proficiency in CPR+ or similar EMR and intake systems.
- Sterile compounding experience is a plus.
Experience- Minimum of two years of experience in a home infusion pharmacy, specifically within the intake process for IVIG therapies.
