The Non-Clinical Intake Coordinator (LVN Clinical Intake Coordinator) plays a critical role in coordinating patient referrals and intake processes for home health services. This position ensures seamless communication between patients, families, physicians, and interdisciplinary teams while verifying insurance coverage and facilitating timely scheduling of services.
This role requires strong organizational and communication skills, along with the ability to manage multiple priorities in a fast-paced healthcare environment. The coordinator also serves as a key liaison to ensure continuity of care, particularly for patients transitioning between care settings.
Coordinate and process incoming referrals for home health services
Verify patient insurance coverage and eligibility
Manage incoming calls from patients, families, physicians, and care teams
Facilitate communication between nursing, therapy, social services, and other disciplines
Assist with staff scheduling and coordination of patient visits
Monitor hospitalized patients and ensure proper follow-up and re-referral upon discharge
Communicate changes in patient status, schedules, and care plans
Support small program coordination with limited scope and impact
Serve as a working lead or resource for team members when needed
Perform additional administrative and coordination duties as assigned
Education: High School Diploma or GED (required)
Licensure: Active Licensed Vocational Nurse (LVN) license (required)
Experience: Minimum of 1 year of relevant healthcare experience
Associate’s or Technical Degree (or equivalent experience)
3+ years of healthcare or home health experience
Prior experience in intake coordination, scheduling, or patient services
Strong communication and interpersonal skills
Ability to manage multiple tasks and prioritize effectively
Knowledge of healthcare workflows, referrals, and insurance verification
Team collaboration and coordination across disciplines
Problem-solving and decision-making abilities
